Claim Forms can be downloaded here, or obtained in hard copy at the NDIC Lagos office Downloaded forms will need to be completed in black ink and taken to the Lagos office.
Help note on downloading Claim forms
1. Right click on the download,
2. Select Save Target As..,
3. Save the document on your computer,
4. Double click on the file you have just saved on your computer
- INSURED DEPOSIT PAYMENT SLIP (NDIC FORM NO. RL-1 )
This form is to be completed in triplicate by all depositors of a closed bank.
- CLAIM FOR PAYMENT OF UNINSURED DEPOSIT (NDIC FORM NO. RL-2 )
This form is to be completed in duplicate by all depositors whose account balances are above -N-50,000.
- UNINSURED DEPOSITORS SWORN STATEMENT FOR INDIVIDUAL (NDIC FORM NO. RL-3 )
This form is to be completed by uninsured individual depositors
- UNINSURED DEPOSITORS SWORN STATEMENT FOR COMPANIES (NDIC FORM NO. RL-4
)
This form is to be completed by uninsured corporate depositors
- INDEMINITY FOR PAYMENT OF DEPOSIT (NDIC FORM NO. RL-5 )
This form indemnifies NDIC and is to be completed by all uninsured depositors
